Before returning the equipment, please remember to submit a Termination Form to terminate your contract.
※ This is ONLY for Sakura Fiber Internet customers who have an active contract with us.
To terminate your contract at the end of the current month, submit a termination form by the 25th of the current month.
※ The contract end date will always be the last day of the month.
※ The NTT equipment must be returned within 1 month from the contract termination date.

How to return the NTT devices (2 options)
Option 1. Return devices using the Return Kit
We will request that NTT send a return kit to your residence.
・The return kit will be delivered to your address in 1 or 2 weeks after your termination form is submitted.
・The return kit will be prepaid.
・The return information is printed on the delivery slip. You do not need to write anything.
Option 2. Return the devices through Sakura Mobile
If you don't have time, you may send us the devices and we will return them to NTT on your behalf.
・The delivery has to be arranged and paid for by yourself.
・The address for our return desk will be emailed to you after we receive your Termination form.
・Once you have shipped the package to our office, please provide us with the tracking information.
※ DO NOT send the devices by Cash On Delivery (COD/着払い/Chakubarai) to our office. We will charge you the delivery fee if we receive the device by Cash On Delivery.

2) Take off the sticker like in the picture to the right |

| 3) Place the sticker on the bag in the blue dotted square | |
| 4) Place the equipment in the brown bag | |
| 5) Take off the paper and seal the bag |

6) Take the bag to one of the following locations: ・Japan Post Office ※Please make sure that the convenience store accepts Japan Post parcels |
