If you are a minor (under the age of 18), a Parental Consent Form is required.

To submit a Parental Consent Form, please check "I am a minor (under 18 years old)" checkbox in the application form. (See the image below:) 

The link to the Parental Consent Form will be sent to your parents'/guardians' email address.

Please ask your parents/guardians to check their email and submit the form on their end.

Please be aware that the Contract Name and registered Email Address will be your own name and email address, not your parents'/guardians'.
